Field Audit Forms IPM Toolbox
To become fully certified, a company must pass a field audit.
Part of the audit includes providing the EcoWise Program Manager
and Field Inspector with a copy of the IPM Toolbox. The Toolbox
is first sent in to the Program Manager along with the Business
Application. A field audit cannot be scheduled without an
IPM Toolbox approved by the Program Manager. This includes
a list of pesticides that might be used in an EcoWise Certified
Service. Toolbox pesticides must meet the EcoWise Certified
Pesticide Evaluation Criteria or be exempted by the EcoWise
Program Manager. These Criteria, along with a list of examples
can be found in Appendix A of the EcoWise Certified Standards.

IPM Standards
The Toolbox that has been approved by the Program Manager
will be reviewed by the Field Inspector. A copy of the IPM
Toolbox Form may be found in the EcoWise Certified Handbook.

The field inspector will use a standardized checklist to evaluate
your IPM Service. For your convenience, a copy of the IPM
checklist can be found in the EcoWise Certified IPM Handbook.

Another field audit form is the Quality Control Form for an
